First Aid Administration: I hereby authorise the staff of the centre and/or present during the period of the event to administer first aid and/or organise medical/hospital treatment as they see necessary, at my expense.

Responsibility: I, and if being a minor, my parent/s, guardian/s, for and on behalf of myself, understand that all sorts fitness & wellbeing will take no responsibility for accident, loss or injury suffered by myself/my child during the course of the activity.

Responsibility within external premises: I, and if being a minor, my parent/s, guardian/s, for and on behalf of myself, understand that all sorts fitness & wellbeing will take no responsibility for accident, loss or injury suffered by myself/my child during the course of undertaking an activity at an external premises. Refund policy: all sorts offer no refunds on holiday camp payments. If notified within 24 hours, a medical make up lesson may be offered. Medical make up lessons are only offered upon production of an original medical certificate as verification of illness/injury and subject to availability. There are no exceptions to this.

Declaration: I declare that the stated person is physically and mentally fit to participate in all sporting activities over the period of the event.

Clothing Attire: All students must wear comfortable clothing appropriate for changing temperatures and must bring a change of clothes.

Payment Methods: Upfront payment for each day is required prior to commencement of all sports camp.

Unavailability of Facility or Services: As a requirement of enrolment, all sports camp attendees/parents/guardians accept that a particular facility or service within the Centre premises may be unavailable at any particular time due to a prior booking, mechanical breakdown, fire, act of God, condemnation, and loss of lease, catastrophe or any other reason. Furthermore, all sports camp attendees/parents/guardians agree not to hold the Centre responsible or liable for such occurrences.

Hours of Operation: The Centre reserves the right to vary, add or eliminate from time to time any of the particular facilities or services provided by all sorts’ fitness & wellbeing. Furthermore, All Sorts Indoor Sports Centre reserves the right to alter lesson schedules and event schedules to suit seasonal and usage requirements.

Conduct within the Centre: Management reserves the right to refuse entry, cancel an enrolment, or request a student/parent/guardian to leave the premises if that person does not behave in a responsible manner, is under the influence of drugs and/or alcohol or does not adhere to the conditions of use. All Sorts Indoor Sports students are not permitted to leave the vicinity unless accompanied by the adult who has signed the child in, unless stated in a written letter that another adult will be pick the child up. Photo identification must be shown upon arrival.

Privacy Policy: All Sorts indoor Sports respects the privacy of all individuals we deal with. In our handling of personally identifiable information about individuals, we comply with the National Privacy Principles standards. Photo Release: This letter confirms the agreement between you and All Sorts Fitness & Well Being Centre, regarding your participation in approved activities in which you may be photographed or videotaped (the Property). For valuable consideration received, you hereby irrevocably grant to the above parties perpetually, exclusively, and for all media throughout the world (including print, nontheatrical, home video, CD-ROM, internet and any other electronic medium presently in existence or invented in the future), the right to use and incorporate (alone or together with other materials), in whole or in part, photographs or video footage taken of you as a result of your participation in approved activities of All Sorts Fitness & Well Being Centre. You hereby agree that you will not bring or consent to others bringing claim or action against the above parties on the grounds that anything contained in the Property, or in the advertising and publicity used in connection herewith, is defamatory, reflects adversely on you, violates any other right whatsoever, including, without limitation, rights of privacy and publicity. You hereby release All Sorts Fitness & Well Being Centre, its directors, officers, successors and assigns from and against any and all claims, demands, actions, causes of actions, suits, costs, expenses, liabilities, and damages whatsoever that you may hereafter have against the above in connection with the Property.
